Who Can Be Admitted
- Recipients with long-term care grades 1–5 who qualify for “facility benefits”
- Those approved by the head of the local city, county or district office:
- Elders referred by an elder protection agency as victims of abuse
- Basic livelihood security recipients or those under emergency measures
What to Bring
- 1. Long-term care certificate
- 2. Individual long-term care plan (issued by the NHIS)
- 3. Resident’s original ID card
- 4. Copy of the guardian’s ID card
- 5. Prescription and doctor’s opinion
- 6. Family relation certificate (for the resident)
- 7. Personal clothing and belongings

Who Can Use the Service

- Care grades 1–4 and grade 5: 8–10 hours per day
- Cognitive support grade: 8 hours a day, 12 days a month
- What is included
- Transport: pick-up at your preferred time
- Available days: Monday to Saturday - Meals: breakfast → snack → lunch → snack → dinner

- 4. Copy of the guardian’s ID card
- 5. Prescription (or doctor’s opinion) from before admission
